Information you choose to share
The consultation form asks for your name, contact details, ZIP code, date of birth, and answers about medications and health conditions. Income and coverage context are optional. You may answer “None” or “Discuss on our call” in the health fields. Please do not send medical records, Social Security numbers, or payment information.
How your request is handled
After you agree to phone and email follow-up, the information completed in each form step is sent to Danielle when you continue to the next step. This lets Danielle respond if you leave before the final booking screen. The final submission sends the complete request. A separate confirmation email is sent to you only after final submission and does not include your health answers. Email is not a dedicated medical-record portal. We do not claim that the wording of a question makes health information non-sensitive.
If you book a call, the scheduling calendar holds your name, email, phone number, and appointment time. It does not receive your date of birth, medication or condition answers, or income. The website does not create a database of your intake answers. Email and calendar providers retain their respective records.
Reviews
The review form asks for a display name, private email address, rating, and written feedback. Submissions are emailed to Danielle and held for moderation. Your email address is used to verify or respond to your feedback and is never displayed with a review. If you give permission and Danielle approves the submission, your display name, rating, and review may appear publicly. Approved lower ratings are not displayed in the selected five-star carousel. Please do not include health details or other private information in a review.
Analytics and page recordings
If enabled and accepted by you, Microsoft Clarity and Google Analytics help us understand use of the marketing page. The intake runs in a sandboxed, isolated frame without these trackers. We do not intentionally send form answers or contact details to analytics. A successful submission can be counted as a general conversion event without personal fields. You can decline analytics or change your choice using “Cookie preferences” in the footer.
Contact preferences
Continuing after the first form step requests a response by phone and email. It does not authorize automated marketing texts. SMS notifications are not active on this site. Ask Danielle to stop follow-up at any time by replying to an email.
Service providers and retention
Website hosting, email, and calendar providers process data necessary to provide these services. We do not sell your form answers. Limited request references and anti-abuse records are kept for up to seven days to prevent duplicate processing. Pending and approved review records may be retained for moderation and publication; declined reviews are removed after 30 days and other review records after one year. Email and appointment records are retained as needed for follow-up, business records, and applicable requirements. Contact Danielle to request access, correction, or deletion, subject to any required retention.
Use by adults
This site is intended for adults seeking insurance guidance. Please contact Danielle directly to discuss coverage for a minor rather than submitting the minor’s personal information online.
